A Case of Pancreatic Acinar Cell Carcinoma Diagnosed after Operation

Abstract

|
Pancreatic acinar cell carcinoma is a very rare disease that is often found incidentally. A 61 year old female presented with abdominal pain. Physical examination and laboratory study were unremarkable, but CT scan showed a 6¡¿4 cm hypodense mass in pancreatic tail with splenic and gastric body invasion. EUS showed a hypoechogenic mass with lobulated margin. The patient underwent distal pancreratectomy with splenectomy and wedge resection of gastric body. Pathology showed a mass with focal necrosis and clear resection margin, and abundant acinar cells with trabecular growth pattern. Immunohistochemical stain showed positivity for PAS, D-PAS, AFB, Mucicarmin, CK, P53 and Ki-67. It was negative for chromogranin. The postoperative course was unremarkable. Final diagnosis was acinar cell carcinoma, moderately differentiated, T3N1M0 (stage IIB).
